- A minimum of $100,000 cargo and $1 million auto liability insurance. (A copy of insurance policy is required)
- Copies/proof provided of the following: motor carriers authority, W-9 with Federal ID number, signed copy of CRST Logistics one-page carrier contract and a completed company profile
